Faculty Qualifications (AA-21) Form Submission Manual

The Faculty Qualifications Form allows for exceptions to the Criteria for Accreditation of the Commission on Colleges of the Southern Association of Colleges and Schools.

System Access Restrictions

The ability to view items and complete actions within Perceptive Content is restricted based on approved system security access.  An FAQ on requesting access updates is available hereQuestions regarding which security group(s) will provide the access needed can be submitted via a Help Ticket.

Submitting a Faculty Qualifications e-Form

Accessing the Form

  1. To get started, open a browser and go to the link, Faculty Qualifications (AA21).
     
  2. The University Web Authentication login screen will display, as shown below:

  3. Enter your NinerNet credentials and select the Log In button.
  4. The AABP Faculty Qualifications (AA-21) Form will display.                                                                                                            

  5. Click the Hide Instructions hide instructionsor Show Instructions show instructionsbuttons to hide or show the instructions.

Completing the Form

  1. The Preparer ID (800#) and Name will automatically be displayed. 
     
  2. Enter the Faculty ID of the faculty member who is requesting an exception and press the TAB key.
     
  3. The Faculty Title, College, and Department will pre-populate in the Faculty Details section. These items are available for update as necessary.faculty details
     
  4. Select the appropriate Semester from the dropdown menu, as shown below:

  5. Select the appropriate Year from the dropdown menu, as shown below:

  6. Select the appropriate Course from the dropdown menu, as shown below:

    Note: Courses available from the dropdown menu are based on Faculty Detail section selections (i.e. semester, year), and the new assignment being entered into the system.

  7. The Course Title and Other Qualifications and Catalog Description will pre-populate automatically. These items are available for update as necessary.

  8. Enter the Academic Credentials, Course Objectives from Syllabus, and Competencies Related to Courses Taught.
     
  9. The Department Chair, College Dean, AABP University Program Specialist, and AA Senior Associate Provost will automatically pre-populate based on the Preparer's information in the Authorization section. These approvers are available for update as necessary.

  10. Enter the UNC Charlotte ID or Name (i.e. Last Name, First Name) of the Business Officer or designated personnel authorized to approve the faculty exception request. 
     
  11. Attach any supporting documentation, such as licenses or certifications, using the attachmentsbutton.
     
  12. Click the submitbutton to complete the e-Form.
     
  13. Approvers will receive an email with an approval task.
  14. Workflow Processors will be able to view, process, and archive submissions.
