What fields are required on the telework form?

There are two Telework Forms:  Telework/Remote Work Agreement  and the  Telework/Remote Work Renewal.   The forms are located on the HR Forms Page - Search by keyword Telework.  

Telework/Remote Work Agreement should be completed to request a telework or remote work arrangement.

Fields required include:

  • Full Name
  • Email Address (For DocuSign, email must be NinerNet@charlotte.edu)
  • Manager Name
  • Manager Email Address (For DocuSign, email must be NinerNet@charlotte.edu)
  • Divisional Vice Chancellor Name
  • Divisional Vice Chancellor Email Address  (For DocuSign, email must be NinerNet@charlotte.edu)
  • Position Type   (EHRA, SHRA, Perm or Temp)
  • Physical Address Where Telework or Remote Work will be performed (Include City, State, Zip)
  • Requested Arrangement Effective Dates  (Start Date, End Date)
  • Explanation of business reason(s) for the arrangement
  • Work Schedule and Location (Day of Week, Work Hours, Work Location)
  • Flexible, Occasional, or Ad Hoc Days (If the schedule is not set, explanation of situation is required)

   

Telework/Remote Work Renewal should be completed when a telework or remote work arrangement is already in place.  Employees should only use the renewal form if there are no changes required to the existing telework/remote work arrangement.

  • Manager Name
  • Manager Email Address (For DocuSign, email must be NinerNet@charlotte.edu)
  • Employee ID 
  • Division
  • Department Name
  • Previous Arrangement Effective Dates (Start Date, End Date)
  • Renewal Arrangement Effective Dates (Start Date, End Date)
