How will the withdrawal of my consent to receive my Form W-2 electronically be affirmed?

 Receipt of affirmation is based on how the consent is withdrawn:

  1. U.S. Postal Service:  The affirmation of the withdrawn consent will be sent via UNC Charlotte email if the email address is known/current; otherwise, a postcard will be sent to the employee.

  2. Email request:  The affirmation of the withdrawn consent will be emailed to the same email address.

  3. Banner Self-Service:  A confirmation will be provided on the screen after the submit button is clicked.
