Employee Separation Form Submission Manual

This document provides general instructions on how to complete and submit an Employee Separation Form.  For specific instructions on the content of the form, please contact your supervisor or department.

System Access Restrictions

The ability to view items and complete actions within Perceptive Content is restricted based on approved system security access.  An FAQ on requesting access updates is available hereQuestions regarding which security group(s) will provide the access needed can be submitted via a Help Ticket.

Employee Separation Form Submission

1.  Open a browser and go to the Employee Separation Form link to launch the form.

2.  When prompted, enter your NinerNet credentials and click Log In.

3.  The form will be displayed with your UNC Charlotte ID and Name auto-populated in the Preparer fields.

4.  In the Separating Employee Details section of the form, enter the Employee ID of the person who is separating from the University, or type their last name in the field and select it from the values that appear.  The remaining fields in that section will auto-populate based on the value you entered.  The UNC Charlotte Phone can be updated on the form, if needed.

Access Restrictions: You may only submit an Employee Separation Form for yourself, an employee that directly reports to you, or employees within your organization (if you are a Business Officer).

5.  In the Separation Details section, enter the Last Day of Work and Access Termination Date.  The position information in this section is auto-populated based on the Employee ID entered in the Separating Employee Details section.

6.  Select a Reason for Separation from the choices in the dropdown.  If Other - Please Explain in Additional Comments is selected as the Reason for Separation, be sure to provide the required explanation in the Additional Comments field at the bottom of the form.

7.  If prompted to do so, provide a response to the question Are you transferring to another North Carolina State Agency?.

8.  The Outstanding Items section will auto-populate items that may require attention prior to employee separation.

9.  Click theattachments  button to add the employee resignation letter.

10.  Click the submit button to save the form to the system for processing.  A pop-up box will appear confirming submission.  If there is a need to clear all values entered and start a new form, refresh your browser window.

Note: Upon submission, confirmation emails will immediately be sent to the initiator, separating employee, supervisor and departments that require related action.  The form can only be re-submitted for a given employee after waiting 14 days from the initial submission.

11.  Exit the form by closing the browser window.
