What should I do if discovering a duplicate PIDM in Banner?

If discovering a duplicate PIDM in Banner, please use this form to begin the process for remediation.

  • The UID#s, usernames, and record names should be entered in the form
    • All fields are required - if information is unknown, enter N/A where needed
    • The form asks to designate records to Keep and Remove - if that designation is not known, enter that detail in the Additional Information field
  • Any customer Personal Information (ie addresses, phone numbers, etc.) should not be entered into the form
