How do I get a university phone line disconnected?

If you are requesting a phone line to be disconnected, please be aware that University
Policy 601.1
states that all permanent employees are required to have a university phone number. Only requests to disconnect phone lines for vacancies will be processed.
Please state that the request to disconnect is due to a vacant position when submitting a ticket to the Service Desk.
Requests for phone line disconnects can only be made if the request is because of a position vacancy.

through the portal
Provide:
a. the reason for the request
b. the extension number
Requests will be processed within 2-3 business days.
